Fee and Payment Policy:
Rising health care cost are a serious problem. Therefore, we want to do everything we can to help control your cost of care and provide our patients with the best care available and with the options for payments.
It is the policy of this office that fees for treatment and/or consultation to be paid in full by the completion of treatment. We do feel that it is the patient's responsibility to know their benefits and be prepared to pay at the time of service. For you convenience we accept MasterCard, Visa, Discover and American Express. For those patients with insurance, we are pleased to assist you in completing and submitting the required forms. Any additional co-pay is due at the time of treatment.
To fit the needs of our patients with extensive or major dental work (Crown, Bridge and Denture work) we offer the Care Credit Plan. Care Credit is ideal for high co-pays and treatments not covered by insurance. They offer a 12 month interest fee plan. An application is available at our office and can be processed while you are here. For more details please contact our Financial Coordinator.
In cases if serious delinquency, treatment will be discontinued, although we will make every effort to accommodate our patients with temporary difficulties.
Appointments and Cancellation Policy:
Every effort will be made to accommodate you and your schedule on a timely basis. However, since unscheduled emergency visits can cause delays we appreciate your understanding.
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice so that we may offer the appointment time to another patient. A fee will be charged for not showing to an appointment without sufficient notice. Thank you for your consideration.
